泌尿科腹腔鏡手術的濫觴,始於Dr. Ralph Clayman在1991年施行的世界第一例腹腔鏡腎臟切除手術。之後二十五年間,泌尿系統腹腔鏡手術蓬勃發展,不論是腎上腺,腎臟,輸尿管,膀胱以及攝護腺手術,皆成功地以腹腔鏡手術完成。腹腔鏡手術相較於傳統開腹手術而言,具有傷口較小,較不疼痛,恢復較快,以及住院天數較短等優點。
 
腹腔鏡手術的方式是在腹壁上建立三至六個手術通道,再使用腹腔鏡手術器械進入體腔內進行手術。在許多需要重建縫合的泌尿科手術中,由於手術器械角度受限,會增加手術困難度。隨著科技的進步,腹腔鏡手術器械有了重大的發展與革新。美國食品藥物管理局於2000年通過達文西機械手臂輔助腹腔鏡手術系統應用於泌尿科手術上。達文西機械手臂系統除了一般腹腔鏡手術的優點外,因器械具有腕關節的高度自由度,在深部縫合上可以如同人類的手腕般靈活轉動,比起一般腹腔鏡手術可達到更好的手術成果。
 
單一切口腹腔鏡手術則是於腹壁上建立單一個三至五公分的切口,所有手術器械皆經由此單一切口進入體腔內施行手術。由於傷口只有一個,比起一般腹腔鏡手術而言可以得到較好的術後傷口外觀。然而也由於器械都經由同一個切口進入體內,器械活動角度受限,手術難度較高。達文西機械手臂系統器械的靈活度,彌補了受限的角度,使得單一切口腹腔鏡手術在機械手臂輔助下能夠更容易地施行。
 
每個人都是獨特的個體,每個病人的病情皆不相同。是否適合接受達文西輔助腹腔鏡手術,或是單一切口腹腔鏡手術,皆需與醫師詳細討論以針對每個人的情況進行最適合的手術與治療。
 
Since the first laparoscopic nephrectomy introduced by Dr. Ralph Clayman in 1991, laparoscopic surgery in urology has increased rapidly for documented benefits of less post-operative pain, better cosmetic result and shorter hospital stay.
 
Laparoscopic surgery is performed via multiple working ports using special instruments. Due to the limitation of instruments, reconstructive surgery is relative difficult. The da Vinci robot-assisted laparoscopic surgery system was approval by FDA for urological surgery since 2000. There is endo-wrist articulation on the instruments of da Vinci system, making it be able to mimic the movement of human wrist, and facilitate suturing and anastomosis in deep body cavity.
 
Laparoendoscopic single site surgery (LESS) means all instruments entered body cavity through a single incision. Comparing to general laparoscopic surgery, LESS has better cosmetic result. However, due to limitation of working angle, LESS surgery is difficult using general laparoscopic instruments. The high degree of freedom of da Vinci instruments makes LESS more easily to be performed.
